How do I schedule an appointment?
Appointments are recommended prior to visiting Altura Centers for Health. You can make appointments via the patient portal, by phone, text, or by walking into one of our health centers.Call or text (559) 686-9097 to get started with our Patient Access team.
Do you take same-day appointments (walk-ins)?
Altura Centers for Health accepts same-day appointments for new and established patients.What should I bring in for my appointment?
Please bring in a valid form of identification with a photo and your most current insurance cards. If possible bring in a copy of your active medications (if any). Please arrive 15 minutes early for your visit to complete patient registration before your appointment with your provider. Please bring your insurance cards, identification and any other information needed for this appointment.What insurances do you accept?
We accept most private insurances, Medi-Cal, and Medicare. We also have Certified Enrollment Counselors to assist with the enrollment into Covered CA free of charge.What if I don’t have insurance?
No person who is unable to pay will be denied services. If you do not have insurance we have various programs that you may qualify for. You may also qualify for the sliding fee scale program based on federal poverty guidelines that requires family size and income. Our Patient Access Representatives can assist you with the enrollment process.Do you provide
transportation?
We offer door-to-door transportation via our private shuttles. Call or text (559) 685-4611 to schedule your transportation pickup.
What if I don’t speak English?
Our staff speak a variety of languages. We offer onsite translation services in all spoken languages, including sign language, through real-time video services.Is my health information
secure?
Altura Centers for Health takes the confidentiality of your health information very seriously. You will be given our Notice of Privacy Practices describing how we use your health information and outlining your privacy rights under HIPAA, the federal regulation governing patient privacy.

Request for Medical Records
Patients can request copies of their medical records by completing our Authorization for Use or Disclosure of Protected Health
Information form and by presenting valid identification. The completed authorization must be signed and dated.

Medical records will not be released without the requester providing proper ID. If you are requesting records for someone other than yourself, you must provide supporting legal documentation such as:
Power of Attorney
Advance Health Care Directive
Living Will
Executor of the Estate
Court order/Conservatorship appointed by the court
